From helping participants develop a support plan that meets their individual needs to providing hands-on assistance, we provide flexible all-round support services to people living with a disability. Every service is designed to help participants get the support they need while striving towards complete independence.

Life stage and transition support

We understand that life changes can be challenging and sometimes overwhelming, even more so for people living with a disability. Life transitions include major changes such as getting married, starting school, graduating, retiring or less positive events such as losing a loved one. We help participants to better understand and deal with such significant changes through active listening, proper planning and empowerment.

Personal activities

With our daily personal care services, participants can continue to live in the comfort of their own home. Our everyday personal support tasks include, but are not limited to: Dressing, showering, shaving, cooking, assisting with medication, helping getting ready for a date, and much more. More than that, we equip participants with the skills needed to live more independently and confidently.

Travel and transport

We help participants get out and about, and empower participants to build their capability to travel independently. Whether that is on their own with personal aids or using public transport. Our travel and transport services can get participants to doctor’s appointments, family and friends visits, shopping sprees, social events, dancing classes, arts and crafts workshops, and many more exciting places.

Community nursing care for high needs

Our specialised and highly experienced support workers deliver nursing care to participants who require more intensive support. This service may be considered if a participants’ needs require challenging behaviour support or complex medical procedures such as home enteral nutrition. Some of the high needs services we deliver are daily management of medications, enteral feeding, complex wound management, tracheostomy care, and urinary catheter management.

Daily tasks and shared living

Our support team can assist participants with everyday tasks in a shared living environment. We help individuals moving into a new place and assist them to accomplish their duties around the house while focusing on building skills that allow participants to live autonomously. Our support also includes assistance with daily tasks such as getting ready for the day, meal preparation, medication management, and moving around the house without having to depend on anyone else.

Innovative community participation

Our Innovative Community Participation Program is designed to give NDIS participants the chance to get more involved in their community, helping participants to feel more connected, build social skills and improve confidence and independence. The program also aims to expand opportunities for employment, allowing participants to demonstrate their unique talents.

Social and civic activities

At MAK Disability & Disability Services we want to create an inclusive environment, giving every person an equal opportunity for personal growth, learning new skills and making new friends. We encourage participants to take part in activities such as workshops or group activities. Our social and civic activities help participants to partake in community, social and recreational activities, like going to the zoo, enjoying a day at the beach, going to the movies, getting creative at an arts & crafts workshop, or discovering native flora and fauna at North Melbourne parks.

Development of daily living and life skills

Our daily living and life skills program is designed to help participants build independence and autonomy. We will work together with participants to look at their personal goals and equip them with the skills, knowledge and confidence needed to achieve their goals. We provide assistance with developing everyday life skills, behavioural skills, social skills, and much more. Our support also includes training in building capabilities to use public transport and travel independently.

Household tasks

We assist participants with daily household tasks that cannot be performed independently. These tasks include things like doing the laundry, cleaning, gardening, vacuuming, collecting mail, bringing out rubbish — and anything else participants need help with around the house. Our staff will sort out all essential household tasks, ensuring that participants can live in comfort.

Mental health support and well-being

Whether participants live with a mental illness or are feeling overwhelmed with everyday life issues, we provide temporary and ongoing support to promote mental health and well-being. Our compassionate and caring staff delivers personalised support services tailored to a participant’s individual needs, so they can achieve their best possible mental health and well-being.
